I know you shake his or her hand when introducing yourself but when you are about to leave do you shake their hand again?

Yes, always shake when leaving too. Thank them for agreeing to meet you or thank them for their time if you have spent more than a few minutes with them. If you say their name (e.g. Mr Smith) whilst shaking hands when you leave, this will help imprint you in their memory too (works better if you have been given leave to use their first name though).
Yes, and say; "It was nice meeting you and I look forward to hearing from you"…or something similar.
